Abstract:Os autores apresentam resumidamente uma série de proposições, justificadas neste mesmo trabalho ou em publicações anteriores, e que se destinam a minorar o sofrimento moral dos doentes de lepra e dos seus familiares e comunicantes, bem como dos casos suspeitos de lepra. Entre as medidas apresentadas contam-se: 1- A suspensão do disgnóstico de lepra em casos benignos, tuberculóides ou indeterminados, diexando-se êstes contudo, sob observação e tratamento. 2- Liberdade de escolha, por parte do doente, dos locais de vigilância, de modo que êle possa fazer-se examinar em localidade distinta daquela em que vive e é conhecido. 3- Redução do tempo de internação ao mínimo compatível com a exigências profiláticas, devendo o doente tratar-se em dispensário sempre que isso for possível. 4- Facilitação da regalia de domiciliar, em condições higiênicas adequadas. 5 - Facilitação de visitas aos hospitais de lepra, principalmente aos comunicantes Mitsuda-positivos. 6- Pavilhões para casos "queimados" em todos os hospitais de lepra. 7- Recomendações aos escritores, poetas, jornalistas, cinegrafistas, oradores, radio-anunciadores e outros profissionais com influência sobre a opinião pública, para que evitem empregar as palavras "lepra" "leproso" em sentido pejorativo e que não usem argumentos em que a lepra aparece excessivamente dramatizada ou de carater sensacional. (Proposção aceita oficialmente pelo Congresso). 8- Organização de um "board" para-oficial constituido de dermato-leprólogos funcionando em seus próprios consultórios particulares, mas em comunicação íntima e constante com o serviço oficial de lepra, e a que os doentes poderão acorrer para maior sigilo ou comodidade. Descreve-se o modo de organização de "board" dessa natureza, que viria regularizar e oficializar uma situação já  existente em muitos países...(AU)^ipt.

Abstract:Toll-like receptor 2 (TLR2) is a key mediator of the immune response to mycobacterial infections, and mutations in TLR2 have been shown to confer susceptibility to infections with mycobacteria. This study investigated the profiles of cytokines, such as interferon (IFN)-gamma, interleukin (IL)-10, IL-12 and tumour necrosis factor (TNF)-alpha in response to Mycobacterium leprae in peripheral blood mononuclear cells (PBMC) with the TLR2 mutation Argo677Trp, a recently reported polymorphism that is associated with lepromatous leprosy. In leprosy patients with the TLR2 mutation, production of IL-2, IL-12, IFN-gamma, and TNF-alpha by M. leprae-stimulated PBMC were significantly decreased compared with that in groups with wild-type TLR2. However, the cells from patients with the TLR2 mutation showed significantly increased production of IL-10. There was no significant difference in Il-4 production between the mutant and wild-type during stimulation. Yhus, these results suggest that the TLR2 signal pathway plays a critical role in the alteration of cytokine profiles in PBMC from leprosy pateints and the TLR2 mutation Arg677Trp provides a mechanism for the poor cellular immune response associated with lepromatous leprosy (AU).

Abstract:Objetico: Determinar a prevalencia de olho seco em portadores de hanseniase do Hospital de Dermatologia Sanitaria de Goiania, comparando-se a um grupo controle. Desenho: Estudo de prevalencia. Material e metodos: A amostra do presente estudo inclui 70 portadores de hanseniase, do Hospital de Dermatologia Sanitaria de Goiania, e 30 individuos no grupo controle, da Fundacao Banco de Olhos de Goias, ambos localizados em Goiania-GO. Foram realizados exeme oftalmologico e testes de Schirmer I, break-up time (BUT) e rosa bengala em todos estes individuos em uma unica avaliacao. Resultados: Quarenta e quatro (63,0%) portadores de hanseniase eram do sexo masculino e 22 73,3%) individuos do grupo controle, do sexo feminino (p=0,001). A idade medica dos hansenianos foi de 61,1+-12,5 anos e no grupo controle, 55,7+-9,6 anos. Quinze (21,4%) hansenianos e quatro (13,3%) individuos deo grupo controle apresentaram diagnostico de olho seco (p=0,429) A forma virchowiana (74,2%) da hanseniase foi a mais prevalente e o olho seco (66,7%) foi mais frequente nesta forma clinica da doenca. Conclusao: A prevalencia de olho seco nos portadores de hanseniase foi semelhante a encontrada nos individuos do grupo controle (AU).

Abstract:The Mycobacterium tuberculosis has a protective cell wall which can be definitely dissolved, we believe, and proven clinically, by the use of esters of chaulmoogra oil. At the present time the more refined preparation of Moogrol is recommended because it is painless, more concentrated and stable. The clinical result of this "softening and conditioning process" over a 30 day period warrant further investigation, not only in the urological tract, but in all types of tuberculosis and especially where there is available an electron microscope to determine the effect of the dissolution on the waxy cell wall of the Mycobacterium tuberculosis. While the present series of cases is small, this study is preliminary and has only prevailed since early 1947, but the in vitro studies, the animal inoculation and the clinical results have been so striking as compared to prolonged hygienic, sanatoria or routine tuberculosis regime as to warrant publication and further investigation. Abstract:1- The lepra bacillus is of feeble pathogenicity, but is able to act as an antigen and stimulate the specific tuberculoid response over a wide range of concentration of the antigen. 2- An effective degree of sensitisation is evidenced by (a) a tuberculoid histology and (b) a positive reaction to lepromin both early and delayed. 3- A primary altent period, the length of which is variable is required to initiate the hypersensitive state. 4- There are two elements concerned in clinical tuberculoid leprosy, (a) A primary focus of infection which sensitives and leads to the production of antibodies; this may be the only infective focus seen, (b) Secondary foci of exogenous or metastatic origin. 5- The essential change on sensitisation is the ability conferred on sensitised tissue as a whole to produce locally at a greatly accelerated rate an effective concentration of antibody in response to the presence of bacilli. The antibody is apparently mainly sessile, but some diffusion is probable. 6- The appearance of secondary spreading foci indicates a previous fall in the antibody content of the skin to an ineffective level, caused by a decline in sensitivity. During this phase bacilli are able to multiply and spread. The resolution of these foci follows the resurgence of sensitivity to an effective level, accompanied by the synthesis of fresh antibody, the exact clinical appearance depending on the time taken for the response to return. 7- The tuberculoid macule follows the return of sensitivity, and spreads as this reaches an effective level over a widening area. 8- The effect of the response is to localise the infection and fix it in thhe sensitised tissue. 9- The response is not an "all or none" response, but varies widely in intensity, depending on the amount of antigen and on personal idiosyncracy. Although hypersensitiveness exhibits many grades, there is a threshold level for effectiveness which varies with the amount of antigen involved, increasing antigen demanding higher levels of response if advancing and multipying bacilli are to be checked. 10- The response though persistent is not of necessity permanent, it may be intermittent, and it may be lost. Its permanence and its effectiveness are influenced by the degree of vitality of the body. 11- Allergy is not equivalent to immunity, but a successful allergic response imparts a degree of local immunity as well as destroying the bacillus. Title:Cortisone in the treatment of reactional conditions in tuberculoid leprosy.

Abstract:Cortisone and DDS were used together in the treatment of reactions occurring in about a dozen cases of kleprosy of the tuberculoid type. Seven severe cases were followed and reported. Two of these were changed to potassium antimony tartrate injections, and one was given that drug for a recurrence of the condition. The other four patients did not require additional treatment for the reaction. In all but one case the edema and erythema could be controlled, but twelve weeks of treatment and an average total dose of four grams of cortisone were required. In addition to the improvement of the skin lesions, there was a slight reduction in the numbers of bacilli. None of the patients developed an exacerbation of neuritis while on DDS and cortisone. There were no untoward reactions, but the relatively high cost of the treatment requires emphasis. (AU).

Abstract:The purpose of this study was to acquire information on the immunological relationship between leprosy and tuberculosis, with respect to the theory that tuberculosis protects against leprosy. The results indicated that the late reaction to lepromin, produced in some tuberculin-negative healthy people was slightly intensified and accelerated in patients with tuberculosis and tuberculoid leprosy, which could be explained by the nonspecific Dienes effect. Gross hypersensitivity cannot be ruled out as an explanation. If cross hypersensitivity in our study were identical with cross-immunity, which is not certain, it would mean that BCG has a protective influence against leprosy....(AU).

Abstract:Mention is made of contradictory opinions of different authors regarding the ocurrence of transformation of established cases of tuberculoid leprosy to the lepromatous form of the disease. Two cases of tuberculoid leprosy which have undergone that transformation after coming under observation are presented. These cases, together with the clinical, bacteriological and histological studies of other authors, seem to agree with the opinion of Manalang that tuberculoid leprosy is a stage in the evolution of leprosy, not immutable as assert by Schujman. It is indicated that such transformation among adult tuberculoid cases is rare because they represent the filtered resistant ones, or benign infections. Continued follow-up and life-long study of contact children born of leper parents in institutions is indispensable if we are to know the evolution of leprosy. (AU).

Title:The antituberculosis and antileprosy activity of disubstituted thiocarbanilides.

Abstract:On the basis of the taxonomic relationship between mycobacteria and fungi, numerous thioureas known to be potent fungicides were tested for their activity against mycobacteria. Many disubstituted representatives of this group of drugs, among them especially certain thiocarbanilides, demonstrated excellent in vitro and in vivo activities against a virulent strain of M. tuberculosis, and Su 1906 showed moderate activity against murine leprosy. Activity of the thiocarbanilides depends on the nature of the substituents in the R and R1 positions. In the example given, those with 3 to 5 carbon atoms possessed most activity. Results of treatment of experimental tuberculosis in mice and guinea-pigs under various conditions are reported. Combinations of suboptimal doses of Su 1906 with suboptimal doses of isoniazid or of streptomycin resulted in considerable enhancement of activity. The tubercle bacillus developed, slowly, only a low grade of resistance to Su 1906, and when the latter was used in combination the rapid and high resistance to streptomycin was appreciably delayed, and that to isoniazid was almost completely suppressed. Su 1906 has been found to have activity against the photochromic "atypical" mycobacteria in some cases more than INH or streptomycin. Not a great deal of stress is laid on the results of treatment of murine leprosy, because that is not a good test object for assessing activity of drugs for human leprosy. Clinical reports on the action of certain thiocarbanilides in human leprosy are encouraging. However, the chemotherapeutic assessment of new drugs, particularly in diseases like tuberculosis and leprosy, requires prolonged observation in all types of the infection. It is not only the immediate effectiveness of a new drug, or its lack of toxicity, or the paucity of untoward side effects, which make its introduction worthwhile. Factors such as persistence of beneficial effects, absence of rapidly-acquired resistance, and compatibility with other types of therapy are of equal importance. For the benefit of those who are in need of this therapy, it is to be hoped that future experience will prove that all the above conditions will be fulfilled by this new thiocarbanilide class of antituberculosis and antileprosy compopunds. Title:Conversion of simple macular neural leprosy to the lepromatous type: a report of three cases.

Abstract:Our observations of these three cases, though incomplete, afford some information regarding the changes occurring when they underwent transformation from the neural to the lepromatous form of the disease. The first one was typically "neurmacular" when first seen, with characteristic lesions that were apparently quiescent and certainly not lepromatous, but during the course of a year they underwent slow, progressive activation, one after the other, and became bacteriologically positive; after that, with "reactional" disturbances of a sort, new ones of lepromatous nature appeared. The second one was almost as definitely neuro-macular when first seen, with a single anesthetic lesion that when biopsied more than a year later was diffusely erythematous (which we would now consider a suspicious condition) but still bacteriologically negative and histologically without suggestion of lepromatous transformation; after another period, somewhat longer, new lesions distinctly of that nature were found, and the patient is now doing far from well. The third case is the simplest up to a certain point, in that when he was first seen the only active lesion (there were two residual ones) was a small recent macule that slowly enlarged for nearly years, when new ones of apparently the same nature appeared; the patient was not seen for three years thereafter, and the events of that period are unknown, but the case has become rather extensively lepromatous...(AU).
